What is housing with integrated support?

Housing with integrated support is a proven, cost-effective, and compassionate housing model that Plymouth uses to help people out of homelessness.

By “housing,” we mean a stable home with no time limit.

By “integrated support,” we mean on-site services tailored to residents’ individual needs that helps them thrive.

A stable, caring place where residents can heal

Each Plymouth resident is paired up with a housing case manager who gets to know them and helps them access services.

PSH residents have access to:

On-site nursing and medical care
Behavioral health treatment
Substance use treatment
Hospice care
Veterans counseling
Family reunification
Money management programs
Community meals, activities and outings

Supporting Resident Health & Safety

Plymouth provides individualized services that support residents’ health, safety and long-term housing stability. Our staff work alongside residents to understand their needs, reduce immediate risks and connect them with resources that can support their well-being.

At the center of our approach is the belief that residents are the experts on their own lives. People set their own goals and make changes when they are ready. Our staff provide compassionate, nonjudgmental support as residents work toward greater health and stability at their own pace.

Individualized care is based on tools, research, and education to minimize risk and maximize safety and accepts the reality that substance use is part of our world.

Our approach recognizes that every resident has different needs. We provide individualized support that prioritizes health and safety, reduces risk and helps residents make choices that support their well-being.
